Census Database Form Instructions
Listed below you will find some very important information that will aid you in completing the database update form.
The Database Structure
Household
The database is designed to store information regarding a member’s residence location. The location (street address, city, state) is referred to as the “Household”.
Household Members
The following terminology is used to describe the members who live in the household.
Head of Household (HOH)
The HOH designation is used to describe an adult member who is considered the head of the household. The database will allow up to two adults to be assigned this designation. For example, if a married couple live in the household, they would be designated as Head of Household – 1 (HOH-1) and Head of household – 2 (HOH-2). It does not matter which one of the married couple is listed first.
Please note that the individual whose name and contact information is provide at the top of the form will be considered HOH -1.
Other Household Members
The following designations are used to describe the other members of the household based on their relationship to one of the household members.
Minor Child: indicates the individual is the minor child of one of the head of household members.
Adult Child: indicates the individual is the adult child of one of the head of household members.
Guest Child: indicates the minor individual living in the household is not the child of either head of household.
Other Adult: indicates the adult individual living in the household is not the child of either head of household.
Important Fields
Email Address
- The email address provided should be the same email address used to make financial contributions to the church.
- Each head of household should have their own individual email addresses. If a couple is sharing an email address, it should be listed for only one of the head of household.
Household Member Names
If your name is Robert and you are commonly called Bob, you need to determine which name you will use when accessing the database.
